Prima Ignis failure at GenCon

At GenCon last week there were reports of the Prima Ignis homunculus ability sometimes not working.  After looking at the code, I did not see any immediate reasons why it should not work, so any details about the situation in which it failed and how it failed will help immensely.

Does anyone who was at GenCon know any more details about Prima Ignis failing? Did it activate and fail, or was it not able to activate at all?  Is it possible it was tried after the homunc had attacked?  As currently written, Prima Ignis and attacking are mutually exclusive, so attacking would then prevent using Ignis in the same turn.

Did it deduct the cost?
Were there any screen messages shown?
Was the target selected by clicking a tile, or via the On-Tile interface in the lower-right?
Was the target a minion, monster, boss, or homunculus?
Were there other cards on the tile targeted in addition to the targeted card?
